Persistent Systems Extends Salesforce Health Cloud to Support Patient and Provider Engagement and Interaction

Persistent Systems (BSE & NSE: PERSISTENT), a global software developer, today announced its support for the new Salesforce Health Cloud, which is generally available for purchase today. Health Cloud gives healthcare providers a platform to make smarter care decisions, engage with patients across their caregiver networks and manage patient data. Persistent Systems joins Salesforce as a Health Cloud launch partner at the Healthcare Information and Management Systems Society (HIMSS16) Conference in Las Vegas.

In partnership with Salesforce, Persistent Systems is launching new solutions for healthcare providers to transform the experience of their customers and members, which improve health care providers’ outreach effectiveness and improve care team productivity, communication and collaboration.

The announcement also coincides with the appointment of healthcare industry leader Jim Rogers, RN, BSN as director of Healthcare Solutions at Persistent Systems. Formerly director at CenturaConnect at Centura Health, Colorado’s largest healthcare provider. In his role, he will be instrumental in helping other healthcare organizations embrace Persistent Systems and Salesforce Health Cloud as a solution for their healthcare and patient engagement needs.

The announcement follows the December release of Persistent Systems’ Physician & Patient Relationship Management Salesforce Fullforce Solution, also leveraging the Salesforce platform.

In partnership with the Salesforce Health Cloud, Persistent Systems enables the following experiences:
Physician Referral: Allows an external physician to refer a patient, see information from past referrals and communicate with all appropriate intermediaries
Physician Liaison: Allows for a liaison to see referral patterns, recruit new physicians and build strong relationships
Care Plan Templates: Create disease-specific templates, automatically creating goals and tasks for the care team
Knowledge Integration: Medline integration to show context-sensitive data (related to diagnosis, medications) inside the Health Cloud Care Console
Nurse Triage Checklists: A customizable checklist for nurses or care coordinators to reference to onboard new patients
Wellness Data Integration: Google Fit data integration with Health Cloud, such as heart rate and daily steps counts to the care team. Workflows provide alerts to the care team when thresholds are not met
Risk Stratification: Allows creating patient cohorts categorized into high, rising, low and other user defined risk criteria, based on Charlson/Deyo Framework.
Oncology Care Model: Improvements in care coordination implemented per guidelines issued by CMW for effective treatment and care of cancer population
EMR Integration: FHIR-based integration using middleware tool – compatible with all EMR’s supporting the FHIRE standard.
